Job Overview:
The Intensive Case Management (ICM) Caseworker plays a crucial role in addressing the barriers to safety and wellness faced by vulnerable populations, particularly refugees and asylees. This position requires a strengths-based, client-centered approach to case management, focusing on single-headed refugee households, survivors of domestic violence, older refugees, youth without guardians, and those with mental health or physical challenges. The ICM Caseworker will collaborate closely with clients to develop tailored service plans that meet their unique needs, utilizing a wide array of resources including financial, medical, and social services. The role demands a proactive approach to client engagement, ensuring that each individual receives the support necessary to navigate complex systems and achieve self-sufficiency.

Duties and Responsibilities:
The ICM Caseworker's responsibilities include determining client eligibility for ICM programming through intake assessments, developing individualized SMART self-sufficiency plans, and evaluating client progress biannually. The caseworker will assist clients in navigating healthcare and social service systems, which may involve applying for benefits and scheduling appointments. Community outreach is essential, as the caseworker must maintain a robust network of service providers to meet client needs.
