Job Overview:
The Family Integrated Case Management Senior Practitioner role at 54 Reasons is pivotal in strengthening families and ensuring children thrive. Duties and Responsibilities:
The duties and responsibilities of the Family Integrated Case Management Senior Practitioner include:

• Building strong, trusting relationships with families to identify their strengths, needs, and goals, and providing holistic, family-centered case management support.
• Conducting thorough assessments and developing tailored support plans that promote child safety, well-being, and positive family outcomes.
• Collaborating with schools, community organizations, and service providers to coordinate support and ensure families are connected to the appropriate services.
• Providing early intervention and practical support to families to mitigate risk factors and enhance family functioning.
• Maintaining accurate case notes, documentation, and reporting in accordance with organizational policies, procedures, and child safeguarding requirements.

Required Qualifications:

• A relevant tertiary qualification in Social Work, Human Services, Psychology, Community Services, or a related field.

• A current Working With Children Check (WWCC) and the ability to obtain a National Police Check.
• A strong understanding of child protection, child safeguarding, and early intervention frameworks.
• A current driver’s license and willingness to travel locally to support families and engage with schools and community services.
