Job Overview: The Family Integrated Case Management Senior Practitioner role at 54 Reasons is pivotal in strengthening families and ensuring children thrive. Duties and Responsibilities: The duties and responsibilities of the Family Integrated Case Management Senior Practitioner include:
Building strong, trusting relationships with families to identify their strengths, needs, and goals, and providing holistic, family-centered case management support.
Conducting thorough assessments and developing tailored support plans that promote child safety, well-being, and positive family outcomes.
Collaborating with schools, community organizations, and service providers to coordinate support and ensure families are connected to the appropriate services.
Providing early intervention and practical support to families to mitigate risk factors and enhance family functioning.
Maintaining accurate case notes, documentation, and reporting in accordance with organizational policies, procedures, and child safeguarding requirements.
Engaging in reflective practice and continuous professional development to enhance skills and knowledge in family integrated case management.
Required Qualifications: The position requires the following qualifications:
Strong expertise in Family Integrated Case Management, with a proven ability to build trust with families and work collaboratively with schools and community services.

Excellent communication and relationship-building skills to work effectively with families, colleagues, and external partners to achieve positive outcomes.
A relevant tertiary qualification in Social Work, Human Services, Psychology, Community Services, or a related field.
Demonstrated experience in case management, family support, or child and family services.
A current Working With Children Check (WWCC) and the ability to obtain a National Police Check.
A strong understanding of child protection, child safeguarding, and early intervention frameworks.
A current driver’s license and willingness to travel locally to support families and engage with schools and community services.
