The American Heart Association has a Part-Time Hourly Community Impact Manager position (10 to 15 hours per week) available in the Western Tidewater area. This position is grant-funded through August 31st, 2025. This will be a Hybrid position: 75% work from home, 25% community based. Please note that this program will be serving the Western Tidewater area, which includes cities of Franklin and Suffolk, and Isle of Wight, Southampton, Surry, and Sussex counties.

The Community Impact Manager is responsible for handling the clinical and operational metrics related to the No Pressure: A Clincial Blood Pressure Program initiative across Western Tidewater, working to improve hypertension control rates. Oversee practice assessment process for up to 10 participating Health Care Organizations (HCOs). Provide outstanding support to HCOs through relationship management in collaboration with local staff and leadership. Responsible for daily operations related to the grant measurables with strong attention to detail. Organize, plan, and complete supportive activities including, but not limited to, regular communications to cohort, sharing of resources, hosting bi-annual cohort meetings, and other CME and non-CME credit bearing events. Focusing on hypertension and clinical operations improvement and implementing policy of systems changes. Engage cohort HCOs to ensure best practices and quality improvement for treatment of patients through the American Heart Association’s outpatient programs: Target: BP, Check. Change. Control. Cholesterol, and Target: Type 2 Diabetes.

Responsibilities

Job Responsibilites:

• Function as the primary contact and resource for internal and external partners and represent the region as the subject matter expert (SME) to introduce and engage funding partners and community leaders with the Association's blood pressure programs. Promote and expand positive community relations through active outreach. Secure and lead the involvement of appropriate community organization(s) including Health Care Organizations to ensure program(s) become self-sustainable community asset(s.

• Lead modification, implementation or instruction in the execution of program curriculum or guidelines. Lead and evaluate the implementation of proposed curriculum.

• Handle the recruitment, training and participation of community partners to develop a network of champions to support the program.

• Provide program and clinical/community partner oversight for each program from initial implementation to ongoing maintenance. Monitor progress, needs and site issues.

• Provide review and reporting for program expenses and evaluate program progress and results to ensure that the program is run within budget.

• Regularly evaluate and report on progress towards stated objectives and provide support for achievement of grant/funding deliverables to support sustained community involvement and program funding. Facilitate the completion of pre- and post-evaluations.

• May recruit, select, orient, handle and evaluate qualified instructors or representatives to efficiently implement the program within assigned sites.

Qualifications

• University/College degree or equivalent experience, preferred. A degree in health promotion, community health, or public health degree a plus.

• Minimum one (1) year work experience in administrative office experience or project coordination experience, with strong attention to detail.

• 1 year of experience with project management from inception to sustainable operations

• 1 year of experience with related health program through study or educational instruction

• Demonstrated ability to work on multiple tasks concurrently.

• Intermediate knowledge and skill with Microsoft Office 365 is necessary. These skills are subject to testing.

• Ability to objectively evaluate, make effective decisions and develop alternative solutions.

• Ability to work in a team environment and interact with all levels of American Heart Association staff, volunteers, and the public. This includes ability and willingness to build and cultivate relationships within and outside the American Heart Association. This also includes the ability to work collaboratively with others to use resources to achieve common goals.

• Ability to be diplomatic, self-motivated, resourceful, considerate, and flexible.

• Knowledge of and skill in report preparation, proofreading and attention to detail.

• Access to reliable transportation and ability to maintain valid driver’s license and auto insurance.

• Must pass background check and must be at least 18 years old.

Preferred Qualifications, not mandatory to qualify :

• Project management experience

• Proficient in Microsoft SharePoint system
