Job Overview:
The Senior Program Consultant, Clinical Implementation will play a pivotal role in leading the clinical implementation strategy for the Cardiovascular Kidney Metabolic Health (CKMH) initiative. This position is crucial for supporting systematic assessments and improvements in clinical practice related to CKM syndrome. The consultant will manage a team, oversee operational processes, and ensure effective data flow and quality improvement activities. The role requires collaboration with various departments within the national office and health system participants to enhance clinical workflows and patient outcomes. The consultant will also provide quality improvement consultation and resources to support change management, ensuring that evidence-based approaches are effectively implemented across assigned markets.
Duties and Responsibilities:
The duties and responsibilities of the Senior Program Consultant include leading the clinical implementation strategy for CKMH, managing a cohort of markets, and providing individualized and group quality improvement consultation. The consultant will be responsible for developing clinical decision support tools, working with interdisciplinary teams, and facilitating learning cohort meetings. Additionally, the consultant will monitor key metrics using the Association’s registries, communicate regularly with the National Director, and ensure that grant deliverables are met. The role also involves building relationships with health system teams, providing ongoing resources for hospitals and clinics, and overseeing the team’s tasks and assignments.
Required Qualifications:
Candidates must possess a Bachelor’s Degree or equivalent experience, along with five to seven years of relevant experience in healthcare or public health. Supervisory experience is required, as well as skills in translating clinical content for non-clinical audiences. The consultant should have a strong understanding of quality improvement methodologies, experience in practice facilitation, and the ability to analyze data for performance improvement opportunities. Strong interpersonal skills, project management experience, and proficiency in Microsoft Office are also essential for this role.
Educational Background:
A Bachelor’s Degree is required, with a preference for candidates holding a Master’s degree in a clinical field or quality improvement certification. The educational background should align with the responsibilities of the position, emphasizing knowledge in clinical practice guidelines and patient engagement strategies.
