Job Overview:
The Program Consultant Healthcare Quality – Post Acute Stroke position is a critical role within the American Heart Association, focusing on improving care for patients with cardiovascular disease, particularly in the post-acute setting. This position is responsible for implementing quality improvement programs in South Dakota, working closely with healthcare providers to ensure adherence to AHA guidelines. The consultant will engage with multiple healthcare sites, providing education and motivation to enhance clinical practices and treatment processes. The role requires effective communication of project statuses and collaboration with various departments to achieve project goals within budget and timelines. This position is home-based, allowing for flexibility while still requiring travel to South Dakota for program implementation and support.

Duties and Responsibilities:
The Program Consultant will drive the implementation of quality improvement projects aimed at enhancing treatment for cardiovascular disease patients. Key responsibilities include:
  • Providing consultation, direction, and support to healthcare providers through learning cohorts to identify and implement strategies for improving patient outcomes.
  • Building and maintaining relationships with internal and external stakeholders, including volunteers and healthcare professionals.
  • Facilitating learning cohort meetings and local conferences in collaboration with volunteers and staff partners.
  • Developing program implementation strategies that align with initiative objectives and ensuring sustainability of grant components.
  • Creating clinical decision support tools and collaborating with clinicians to improve care delivery.
  • Recruiting program participants and working with clinical care teams to address barriers to guideline adoption.
  • Communicating regularly with the Senior Manager to ensure tasks are completed according to grant timelines.
  • Managing project documentation, including budgets, timelines, and status reports for both internal and external audiences.

Required Qualifications:
Candidates must possess a Bachelor’s Degree or equivalent work experience, along with a minimum of 2 years of relevant experience in a healthcare or public health-related organization. Experience in a post-acute clinical setting, particularly related to stroke care, is preferred. Proficiency in project management is essential, including the ability to manage budgets, timelines, and communication with stakeholders. Strong verbal, written, and presentation skills are required, as well as the ability to work effectively in a team environment. Comprehensive skills in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, are necessary. Candidates should also be willing to travel up to 15% of the time, including overnight stays in South Dakota.

Educational Background:
