Job Overview:
The Program Consultant Healthcare Quality – Post Acute Stroke position is a full-time, grant-funded role that supports the implementation of quality improvement programs aimed at enhancing care for patients with cardiovascular disease, specifically in South Dakota. This position is home-based, allowing for flexibility while working closely with various healthcare providers to educate and motivate them to adhere to clinical guidelines and improve treatment processes. The consultant will be responsible for managing project timelines, budgets, and deliverables, ensuring that all initiatives are executed effectively and within the established parameters. The role requires collaboration with multiple departments within the national office and engagement with stakeholders to drive successful outcomes in healthcare quality improvement.

Duties and Responsibilities:
The primary duties of the Program Consultant include driving the implementation of quality improvement projects that align with American Heart Association guidelines for treating cardiovascular disease. This involves providing consultation and support through learning cohorts, developing program implementation strategies, and facilitating meetings and conferences. The consultant will also be responsible for creating clinical decision support tools, recruiting program participants, and working with clinical care teams to address barriers to guideline adoption. Regular communication with the Senior Manager is essential to ensure tasks are completed according to grant timelines, and the consultant will manage project documents, including budgets and status reports for both internal and external audiences.

Required Qualifications:
Candidates must possess a Bachelor’s Degree or equivalent work experience, along with a minimum of 2 years of relevant experience in a healthcare or public health-related organization. Experience in a post-acute clinical setting, particularly related to stroke care, is preferred. Proficiency in project management, including budget and timeline management, is essential. Strong communication skills, both verbal and written, are required, as well as the ability to work collaboratively with diverse teams and stakeholders. Comprehensive skills in Microsoft Office Suite are necessary, and candidates should be willing to travel up to 15% for work-related purposes.

Educational Background:
A Bachelor’s Degree in a relevant field is required, and equivalent work experience may be considered in lieu of formal education. Candidates with additional certifications or training in healthcare quality improvement or project management will be viewed favorably.

Experience:
