Job Overview:
The Program Consultant Healthcare Quality – Post Acute Stroke is a pivotal role within the American Heart Association, primarily focused on enhancing the quality of care for patients with cardiovascular diseases, particularly in the post-acute phase following a stroke. This position is responsible for implementing quality improvement programs in South Dakota, working closely with healthcare providers to ensure adherence to AHA guidelines. The consultant will engage with multiple healthcare sites, providing education and motivation to improve clinical processes and practices. Duties and Responsibilities:
The Program Consultant will be tasked with driving the implementation of quality improvement projects aimed at helping healthcare providers align their practices with American Heart Association guidelines for cardiovascular disease treatment. Key responsibilities include providing consultation and support through learning cohorts, developing program implementation strategies, and establishing sustainability plans for grant components. The consultant will facilitate learning cohort meetings and local conferences, build relationships with stakeholders, and develop clinical decision support tools. Additionally, the consultant will recruit program participants, work with clinical care teams to address barriers to guideline adoption, and manage project documentation, including budgets and timelines. Regular communication with the Senior Manager is essential to ensure that tasks are completed according to grant timelines, and the consultant will also be responsible for reporting project statuses to both internal and external audiences.

Required Qualifications:
Candidates must possess a Bachelor’s Degree or equivalent work experience, along with a minimum of 2 years of relevant experience in a healthcare or public health-related organization. Experience in a post-acute clinical setting, particularly related to stroke care, is preferred. Proficiency in project management is essential, including the ability to manage budgets, timelines, and communication with team members and stakeholders. Strong verbal, written, and presentation skills are required, as well as the ability to work collaboratively in a team environment. Comprehensive skills in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, are necessary. The role may require travel up to 15%, including overnight stays in South Dakota.

Educational Background:
