Job Overview:
The NCD HeartCare Outreach and Data Focal Person plays a crucial role in supporting the Ministry of Health's NCD team in planning and coordinating outreach activities related to non-communicable diseases (NCDs). This position involves analyzing and presenting HeartCare data, enhancing data quality, and ensuring the follow-up of high-risk patients. The focal person will facilitate the transition from mere data collection to practical application, transforming HeartCare data into actionable outreach plans, patient follow-up lists, and performance summaries. The role is integral to the day-to-day operations of the NCD team, focusing on outreach coordination, data analysis, and routine program reporting, thereby contributing to improved health outcomes in the community.

Duties and Responsibilities:
The duties and responsibilities of the NCD HeartCare Outreach and Data Focal Person include:

• Supporting NCD outreach planning and coordination by preparing outreach plans, schedules, and activity trackers.
• Liaising with the NCD National Coordinator and various teams to confirm outreach logistics.
• Utilizing HeartCare data to identify priority communities and prepare patient lists for outreach.
• Tracking outreach completion and reporting any delays or cancellations.
• Analyzing and summarizing HeartCare data for program action, including preparing reports and presentations for management.
• Supporting the identification and follow-up of high-risk patients, maintaining a tracker for follow-up activities.
• Conducting routine data quality checks and providing feedback on data issues.
• Preparing user support plans and assisting health workers with HeartCare workflows.
• Submitting regular reports on outreach activities and maintaining coordination documents. 1
• Coordinating with the Ministry of Health, WHO, and partners to ensure alignment with health priorities and effective data management.

Required Qualifications:
The required qualifications for this position include a university degree in public health, health information systems, health sciences, nursing, data management, statistics, or a related discipline. A postgraduate qualification in relevant fields is desirable. Candidates must demonstrate at least five years of experience in health program coordination, outreach activities, or public health services. Familiarity with routine health data and experience with digital health systems are also essential. Strong communication, problem-solving, and teamwork skills are necessary for effective collaboration with various stakeholders.

Educational Background:
Candidates must possess a university degree in public health, health information systems, health sciences, nursing, data management, statistics, or a related discipline. A master's degree in public health or a related field is preferred. Educational qualifications must be from accredited institutions recognized by the World Higher Education Database (WHED).

Experience:
Candidates should have a minimum of five years of relevant experience in supporting health program coordination, outreach activities, or community-based health initiatives. Experience with NCD programs, digital health systems, and data analysis tools is highly desirable. The ability to work with diverse stakeholders and support users with varying levels of digital literacy is crucial.

Languages:
Fluency in English and Bislama is essential for this position. Proficiency in additional languages may be considered an asset but is not mandatory.

Additional Notes:
The salary for this role ranges from VT 2,510,000 to VT 3,102,000 per year, depending on qualifications and experience. The initial contract duration is up to 5 months, with the possibility of extension. This position is subject to local recruitment, and candidates must be Vanuatu nationals residing in the local commuting area.
