Job Overview:
The Community Program Director, Health Equity, will play a crucial role in advancing health equity initiatives within the community. This position involves collaborating with health centers, community organizations, and stakeholders to address chronic disease factors, particularly hypertension, and to promote health literacy and access to healthcare. The Director will lead efforts to implement community-based blood pressure screening and education programs, ensuring that these initiatives are sustainable and impactful. The role requires a proactive approach to engage with diverse communities, assess their needs, and develop tailored action plans that address health disparities. The Director will also be responsible for training and empowering local organizations to enhance their Health Promotion capabilities, thereby fostering a culture of wellness and health equity.
Duties and Responsibilities:
The Community Program Director will have a wide range of responsibilities, including:
- Leading the establishment of community-based blood pressure screening and education initiatives.
- Ensuring the implementation of self-monitoring blood pressure programs and Food Security screenings in collaboration with health centers and community organizations.
- Designing and facilitating training sessions and events for community organizations to enhance their capacity to promote health and wellness.
- Engaging with HRSA-funded health centers to improve blood pressure management and address barriers to care.
- Conducting community needs assessments and facilitating discussions to identify health disparities and develop action plans.
- Recruiting and managing a diverse group of volunteers to support the AHA's mission and strategic objectives.
- Collaborating with local leaders and stakeholders to create a cross-sector engagement council.
- Aligning local initiatives with the AHA's broader advocacy and quality agenda.
- Measuring and reporting on the effectiveness of programs and initiatives to ensure continuous improvement.
Required Qualifications:
Candidates must possess at least 3 years of relevant experience in community or public health, particularly in managing health promotion programs. Proven skills in volunteer recruitment and management, including working with high-level executives, are essential. The ideal candidate will demonstrate critical thinking abilities, project management skills, and the capacity to work effectively in a multidisciplinary team environment. Strong communication skills and the ability to influence others without direct authority are also required. Additionally, candidates must be willing to travel locally up to 75% of the time and have access to reliable transportation.
Educational Background:
A Bachelor's degree in public health, health promotion, or a related field is preferred, although candidates with equivalent experience may also be considered. Some college education combined with relevant experience is also acceptable. Knowledge of Health Systems and social determinants of health is advantageous.
Experience:
The position requires a minimum of 3 years of experience in community or public health roles, particularly in Program Management and health promotion. Experience working with multicultural communities and understanding the social determinants of health is preferred. Candidates should have a track record of successfully managing large projects and events, as well as experience in volunteer management.
Languages:
While no specific language requirements are stated, proficiency in English is mandatory. Knowledge of additional languages may be beneficial, particularly in multicultural communities.
